A critical access hospital dedicated to providing compassionate, community-centered healthcare to residents west of the Brazos in Brazoria County, Texas. The hospital offers a comprehensive range of services, including emergency medicine, inpatient care, primary care, diagnostic imaging, physical therapy, assisted living, laboratory services, general surgery, cardiopulmonary care and wound care.
Challenge
Sweeny Community Hospital (Sweeny) was undergoing a significant organizational transformation as it prepared to move operations into a new healthcare facility. In parallel, the hospital was introducing additional clinical services designed to enhance patient care and expand its service footprint. This dual initiative introduced complexity across clinical, operational and revenue cycle functions, requiring careful coordination to ensure readiness at go‑live. This transformation required a comprehensive review and update of the hospital’s Oracle Health | Cerner revenue cycle platform to ensure alignment with the new services and operational workflows. To support this effort, Sweeny needed an experienced partner to lead the design, planning and development of detailed documentation required for coordination with Oracle Health | Cerner to implement changes within the CommunityWorks system. Additionally, the initiative required integration design, planning and documentation to support connectivity with multiple third‑party systems.
The Accomplishments
- More than 50% of project effort was devoted to partnering with the Sweeny Pharmacy team to address complex pharmacy system remediation. This work drove major formulary improvements, ultimately requiring a full rebuild to standardize medication data and ensure accuracy, safety and downstream integration readiness.
- Partnered with Oracle Health | Cerner to successfully establish and operationalize new services including a Bone Density imaging room and Intermedicate Care Unit (IMU), ensuring proper system configuration, workflow alignment and clinical readiness.
- Led and supported critical system adjustments to address and resolve clinical service gaps within Cerner, including Sepsis, DVT and Falls, Mental Health, Cardiopulmonary workflows, VTE orders and other critical clinical protocols.
- Assisted with system and operational readiness for new third-party integrations, including GE Lab, Blood Gas, GE MUSE, Telemetry and Provation, ensuring alignment with core clinical and ancillary workflows.
- Evaluated SurgiNet requirements, including detailed reviews of procedure-file extracts and supply-chain system extracts, to address configuration gaps and support surgical case accuracy.
- Addressed a broad range of Health Information Management (HIM), Coding, Billing and Accounts Receivable (AR) follow-up system corrections and enhancements to improve data integrity and revenue cycle performance.
- Logged and resolved 300+ Oracle Service Requests (SRs), playing a key role in addressing critical system and operational needs while providing ongoing knowledge, experience and support to the new IT Director and team.
- Implemented 3 net-new interfaces: MUSE (Cardiology), GE Mobile Vital Carts, Endoscopy.
